The Transforming Clinical Practice Initiative (TCPI) is designed to help clinicians achieve large-scale health transformation. The initiative is designed to support more than 140,000 clinician practices over the next four years in sharing, adapting and further developing their comprehensive quality improvement strategies. The initiative is one part of a strategy advanced by the Affordable Care Act to strengthen the quality of patient care and spend health care dollars more wisely.

The TCPI broadly advances practice transformation and participation in payment reform by delivering educational support and training to providers through Practice Transformation Networks (PTNs). PTNs provide quality improvement tools, education, and training, and Support and Alignment Networks (SANs) provide foundational support to the PTNs.

In collaboration with the Practice Transformation Networks, Qualis Health will evaluate and assess providers’ progress by: conducting baseline and follow up assessments; assisting with recruitment of small practices, rural providers, and those that serve medically underserved communities; engaging in learning and diffusion activities; delivering technical assistance; and collaborating with Practice Transformation Networks to support practices needing further transformation support.
