About the Stepping Stones Project

In 2008-2011, Qualis Health led one of Medicare's early demonstration projects aimed at improving care transitions and reducing rehospitalizations. (Learn about our current work on this topic.)

Stepping Stones: Bridging Healthcare Gaps (The Care Transitions Project of Whatcom County, WA) aimed to engage patients, providers, and the community to eliminate unnecessary readmissions to PeaceHealth St. Joseph Medical Center.

As illustrated below, the project's key strategies were centered on the needs of the patient.

Project Results

Despite beginning with one of the lowest 30-day readmission rates in the nation, the project resulted in a 9% decrease in 30-day readmissions. Download our Final Report to the Community.

The Stepping Stones project was examined in a 2013 issue of the Journal of the American Medical Association (JAMA) as part of an article focusing on QIO-led care transitions initiatives nationwide.
