During patient transfers:
How often does the following get communicated?

✓ Known infection with multidrug resistant organisms
✓ Influenza and pneumococcal immunization status
✓ Antibiotic prescriptions and indications

Complete, accurate data relevant to infection control and prevention should be transferred between care settings—every patient, every time. Begin to evaluate your organization's performance by taking these two steps:

  • Confirm that organizations are receiving the information as intended. Follow up with a sampling of organizations to whom you refer patients. Do they actually receive the information? Is it timely? Is it in a format they can use?
